Before your visit
Tell the office what you need
Choose the kind of visit that seems closest and add a short scheduling note if it will help. Do not include health, insurance, payment, or other sensitive information in the form.
Ask before sending records
If another dental office has recent X-rays or records, ask North Jefferson Dental where and how they should be sent before arranging the transfer.
Have your insurance question ready
Keep the name of your insurance company and plan nearby. The office can explain what information it has, and your insurer can confirm benefits and network details.
Ask about costs before treatment
Once Dr. Foreman has examined you and discussed a recommendation, ask what the proposed care may cost, what alternatives exist, and what portion may remain your responsibility.
